In order to complete the underwriting of your application(s) Tyrpak Financial Associates, Inc., its agents and staff, affiliated companies and/or entities, insurance companies and their reinsurers are hereby authorized to possess and/or obtain your existing medical records for the purpose of the procurement of life, health, disability income, long term care, or other insurance or investment products.

The information contained in your medical records for insurance applications will be held in the strictest of confidence and may be used only for the purpose of the procurement, or the evaluating or underwriting for the possible procurement of life, health, long term care, or other insurance products. The contents therein may be reviewed and assessed by a qualified staff consisting of medical directors, underwriters, underwriting assistants, or other related employees involved in the submission, receipt or evaluation of insurance applications or prospective applications.

These records may be transmitted via U.S. regular mail, through various overnight mail services and through the use of secured electronic devices. In all such situations, we stress the confidential nature of information being shared.

We retain records relating to professional services that we provide so that we are better able to assist you with your financial needs, and to comply with various company and industry-required guidelines.
